The Consortium in partnership with the National Council for Palliative Care (NCPC) are looking to better understand the needs of lesbian, gay, bisexual and trans people around death and dying and whether there are any improvements that could be made in end of life care provision.

Who should complete the questionnaire?

The Consortium is asking its member organisations and groups to complete the questionnaire using their experience of working with LGBT people.  Though anyone with either personal experience or experience from the work they do with LGBT people is invited to share their thoughts.

How will the information I give you be used?

None of the data collected will be used to identify individuals, but to identify needs to be considered by end of life care providers.

This survey was written by the Consortium, but based on a similar survey produced by the National Council for Palliative Care.

The Department of Health is funding this work.
